You would have to write down the exact sequence of clicks that lead up to that situation for us to figure it out.

The last try I've done this morning.

I go to the sandbox folder.

I click on add survey.

I fill title, text, authorisations.

I click on question with one answer

I close the 3rd answer and put Yes and No in the 2 first boxes.

Then I click on Ok.

At the bottom of the browser page, I see Done.

The page is blank and remains opened.

In the folder window behind the entry is not created.

I click on the red cross up right. So the windonw is closed and the entry created.

You should file a bug report on this.
